How it Works

Submit either a Quick Scripts Request ($85 option) for a one time prescription, x-ray, or lab request or become a member with an annual membership for yearly refills by filling out the Exquisite Member Health Questionnaire ($175/annual fee plus $40 add on options throughout the year). See below for details.

Within 24 hours the health recommendations will be emailed back with options for you to choose from. This will include prescription medication/refill options, lab tests, preventative health recommendations, etc. (We are able to refill most medications previously prescribed, with the exception of controlled substances for pain/anxiety.)

Pay for service through easy secure checkout.

Prescriptions will be sent to your pharmacy and lab test/X-ray orders will be sent with instructions on where to go next.
Costs
**We do not bill insurance, but receipt may be submitted to your HSA, HRA, or FSA for reimbursement.**​​
Options to choose from:​

OPTION 1: Quick Scripts Urgent Care Cost $85 one time medication prescription, and/or lab/x-ray cost (billed separately) if needed. Just fill out a Quick Script Form and submit.

​​​OPTION 2: ​Annual Membership Cost $175. This covers the Health Questionnaire and personalized health recommendations, plus 1 year of prescription medication refills. Additional medications may be added throughout the year at the membership discount through- Annual Membership Discount-add on medications, lab &/or X-ray orders throughout the year cost $40/each (Requires annual membership.) We are set up with Express Labs with locations available throughout Southeast Idaho and Boise. You may also use a lab of your choice. ​

Add on examples include: urgent care type conditions, not on the original personalized health plan (ie Labs, X-rays, prescriptions for UTI's, hives, antibiotics, etc).

OPTION 3: Free preventative screening orders 24/7. Just fill out the health questionnaire; request order for mammogram, colonoscopy, or cholesterol testing and submit.

**Note prescription medication, labs and X-rays if needed will be billed separately through the company where you have the tests ran and incur a separate charge. These may be ran through your insurance.**
